Required Forms for Twirlers-to-Be
Registration Form
All twirlers-to-be are required to fill out a registration form prior to participating in the classes. There is a $20 nonrefundable fee that will also be due at the time of registration. This form should promptly be shared with either Gina Stropkaj or Loretta Worsham.
Activity/Participation Agreement
All twirlers-to-be are required to fill out an activity participation agreement form prior to attending classes. This form can be filled out by either the participant or authorized guardian. This form should promptly be shared with either Gina Stropkaj or Lorette Worsham.
Required Forms for Current and Twirlers-To-Be
Handbook Acknowledgment
All current twirlers and twirlers-to-be are required to fill out the handbook acknowledgment form prior to participating in the classes. The handbook can be found by clicking HERE and should be read with your twirler. This form should promptly be shared with either Gina Stropkaj or Loretta Worsham.
Photo Release
All current twirlers and twirlers-to-be who are comfortable with their twirler's photos taken and shared should fill out the form. This form should promptly be shared with either Gina Stropkaj or Lorette Worsham.
Non-Required Forms
Uniform Permission Slip
This form IS required if your twirler wishes to wear the Joyful Twirler uniform for an event that is NOT associated with the Joyful Twirlers. This includes performances outside of those organized by the Joyful Twirlers, such as talent shows. Once the form is filled out, Gina Stropkaj or Loretta Worsham will review the request and either allow or deny the request.
